Aqui Para Ti (APT)/(“Here for you”) is a comprehensive, primary care clinic-based, healthy youth development program that provides medical care, coaching, health education, and referrals to Latino youth ages 10 to 24 and their families. APT uses a confidential, family-centered approach, protecting youth privacy while encouraging family members to work together to support the healthy development of the child. This approach, honors familism, and leverages family strengths and skills.
The APT intervention functions as a “clinic within a clinic” where intensive adolescent care is delivered by a multidisciplinary team. This program is funded by the Eliminating Health Disparities Initiative of the MN Department of Health and Hennepin County Medical Center. We also offer to Latino parents of adolescents, a support group called “Café entre Padres” to help them to improve their parenting skills. Other fun and interactive activities that APT provides are the summer activities for youth like Red Cross Babysitter Certification, mentoring program to explore the medical filed, and connections with several organizations in the community. Another unique resource that APT offers is the, School/college connector services to youth and their family that need help to navigate the education system.
The cost of is the same as a regular medical appointment. In addition to the regular appointment, the patient also has access to free health education, mental health evaluation, counseling, and referrals to other resources. If you do not have insurance, we will work with you to find a program to cover your care.
